We proudly accept insurance and multiple payment options for your convenience.
Radiant Health Massage LLC is an approved provider under the VA Community Care Network (VA CCN). Veterans who are referred by their VA doctor can receive covered massage therapy sessions through the VA Community Care Program.
Insurance Providers
We accept most major insurance providers, including:
Aetna, Blue Cross Blue Shield, Cigna, Humana, United Health Care, Personal Injury, and Workers’ Compensation.
We also welcome VA Community Care referrals for qualified Veterans.
If you are a Veteran, please speak with your VA Primary Care Provider or VA Community Care Coordinator to request a referral for massage therapy with Radiant Health Massage LLC.
Once authorized, the VA will send us your referral directly and you’ll be scheduled promptly.
Verification of Benefits
We offer a free service to verify your insurance benefits.
Our team will contact your insurance provider to confirm eligibility and coverage for massage therapy.
If your policy covers our treatments, we will bill your insurance directly.
If not, you may still enjoy our services using cash, credit/debit card, PayPal, or Afterpay.
Self-pay clients can also save on treatments by purchasing a package or membership plan.

New Client Forms
If you’re a new or returning client with private insurance or self-pay, please complete our Acceptance Form.
We will verify your coverage and contact you once your benefits are confirmed.
Veterans using VA Community Care:
You do not need to complete this form directly.
Your VA doctor or Community Care Coordinator must send a referral authorization through the VA system (HealthShare Referral Manager - HSRM).
Once received, we will contact you to schedule your appointments.
